Mobile phone number

I agree to receive transactional SMS messages from Alcon CRS, LLC, doing business as Doxlair, at the mobile number provided. Messages may include six-digit verification codes I request and secure Daily Interview links requested by an authorized account administrator or sent under a workday schedule they enable for the assigned Project Manager. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. Consent is optional and not a condition of purchase. See the Terms of Use and Privacy Policy.

The actual checkbox is unchecked until the user affirmatively selects it. SMS enrollment is optional; verification codes are sent only when requested, and Daily Interview links are sent only when requested or when an authorized administrator enables a project schedule. This sample checkbox is disabled.
